Liaising with mental health teams, they have their shit down. From admin to top brass, but bottlenecks are everywhere. It's extremely disheartening

All face to face appointments are wading through enormous backlogs. Drug service cuts are leading to ever more recidivism. Makes the news for a day, gets punted into touch by Covid cycle

I don't know where we'll be a year from now. I.V cocaine use has pushed Glasgow's hiv rates up further and there have been so may regular needles/work detritus on our dog walk we had to see sense and change it. Across multiples of sites in Nottingham. Grim

Alcohol referrals are up, benzos up although an old skool speed freak was in touch. You rarely see that set any more, the mulching base-heads of yesteryear.

@WashYourHands have any experience with people having to resort to citizen science or even black markets to get their drugs? Something I saw recently said that there is an established insulin market within Monero, a cryptocurrency that is built around anonymity and untraceability, and which dons an IRS-placed bounty for anyone who can "crack" it.

@Clinamenic Citizen science can be like a client who thought he could cold-freeze a filter by separating out codeine from a paracetamol mix, over the counter like Tylenol. Except he fucked it over and over again and now has organ failure. Or the crack addict who after a massive week long session hallucinated that injecting his sister's insulin needles into his knees (serious) would be a huge rush. Except he ended up with a double leg amputation, went blind and now needs 24/7 nursing care. Or the younger more physically robust addicts who push harder on their dose because they know more agencies are carrying Naloxone and qualified to administer

People here can buy from anywhere/everywhere, a great deal of addiction earnings are from sex work (extremely vulnerable end of the spectrum) and drug phone groups within small networks are key. Difficult to infiltrate. Everyone knows the drug snob pyramid anyway and county lines have been in operation since the 1970's. I fuckin despise that term, the deliberate dissociation from the truths it provides and hides

That's the problem @Clinamenic , users here are just like anywhere else. Seeking oblivion. Benzos are a monstrous detox, much more prolonged than heroin and wedging is difficult when people are constantly tightrope walking the heaviest possible nod. Britain is flooded. Supply is raging and with benzos if you're young and bored they're really cheap too

Fentanyl and Carfentanyl are like wackamole - a batch comes in, hits 4 or 5 cities, cleaves a path of death, then the supply dwindles until a few weeks later

I read that at one point they were going to stop offering the option of suboxone/subutex and so the only choice for people wanting to come off smack would be methodone. Did they do that in the end?

From what I've heard, kicking methadone is at least as hard as actually kicking smack itself, so I don't know what advantage it's supposed to have over simply giving addicts heroin instead, which if it's regulated at least isn't going to contain brick dust and god knows what else, or suddenly be ten times stronger than it was last week, with no warning.

There was a study done years ago showing that dihydrcodeine is as effective as methadone for helping heroin addicts get clean, as well as much safer and cheaper, but I don't know if it ever got past the pilot stage: https://www.sciencedaily.com/releases/2006/12/061208101646.htm - thoughts, @WashYourHands?

I wouldn’t call Df’s getting clean and they’re more likely to come as tablets, whereas supervised on-site methadone administered as a liquid application, wedged down, can be enough in tandem with therapy (trauma screening etc) for the committed

Unfortunately, you have to show that you’ve wedged down on a specific compound before addiction services take over full supervision. I don’t like the ethics here as plenty of users want to stop, but the volumes mean you have to screen for the most committed too. I pity the people making those decisions
